CLINICAL PHARMACOLOGY General Cetirizinr is a potent antihistamine with a low potential for drowsiness at pharmacologically active doses and has additional anti-allergic properties. It is a selective H1 - antagonist with negligible effects on other receptors and so is virtually free from anti-cholinergic and anti-serotonin effects. Ctirizine inhibits the histaminemediated early phase of the allergic reaction and also reduces the migration of inflammatory cells and the release of certain mediators associated with the late allergic response. Peak blood levels in the order of 0.3 g ml are attained between 30 and 60 minutes following the administration of a 10 mg oral dose of cetirizine. The terminal half-life is approximately ten hours in adults and six hours in children aged between 6 to 12. This is consistent with the urinary excretion half-life of the drug. The cumulative urinary excretion represents about two-thirds of the dose given for both adults and children. The apparent plasma clearance in children is higher than that measured in adults. A high proportion of cetirizine is bound to human plasma proteins. Pharmacokinetics Bioequivalence Study Since both the test product and the essentially similar product are liquids, no formal bioequivalence studies are required.

Information regarding acute overdosage is limited to experience with cetirizine alone and the marketing history of pseudoephedrine hydrochloride. Overdosage has been reported with cetirizine. In one adult patient who took 150 mg of cetirizine, the patient was somnolent but did not display any other clinical signs or abnormal blood chemistry or hematology results. In an 18-month-old pediatric patient who took an overdose of cetirizine approximately 180 mg ; , restlessness and irritability were observed initially; this was followed by drowsiness. Should overdose occur, treatment should be symptomatic or supportive, taking into account any concomitantly ingested medications. There is no known specific antidote to cetirizine. Xetirizine is not effectively removed by dialysis, and dialysis will be ineffective unless a dialyzable agent has been concomitantly ingested. The acute minimal lethal oral doses in mice and rats were 237 and 562 mg kg, respectively approximately 95 and 460 times the maximum recommended daily dose in adults on a mg m2 basis ; . In rodents, the target of acute toxicity was the central nervous system, and the target of multiple-dose toxicity was the liver. In large doses, sympathomimetics may give rise to giddiness, headache, nausea, vomiting, sweating, thirst, tachycardia, precordial pain, palpitations, difficulty in micturition, muscular weakness and tenseness, anxiety, restlessness, and insomnia. The significant effects of diphenhydramine used as internal control validated the sensitivity of CFFT as a measure of CNS impairment. There was no noticeable difference between the post dose Critical Flicker Fusion scores following fexofenadine, cetirizine and those found with placebo. Diphenhydramine produced highly significant results with peak effect at 3hrs on SSS. The effect with cetirizine on the scale though insignificant, started at 1 hour, became statistically significant at 3 hours and persisted upto 6 hours. Fexofenadine and placebo did not produce any significant effect on SSS at any of the trial periods as followed upto 6 hours. Fexofenadine, cetirizine and placebo did not produce any significant change in the number of digits correctly substituted whereas with diphenhydramine the results were statistically significant on performing DSST. No significant change was observed in number of digits cancelled in DCT after giving fexofenadine, cetirizine and placebo as studied upto 6 hours. The higher incidence of apoptosis in the larger clones 6 ; , the clones that mostly produce the differentiated A1 spermatogonia, suggests that the primary effect of T may be to inhibit the differentiation of undifferentiated spermatogonia to A1 spermatogonia. If this is the case apoptosis may be a secondary process that occurs when the cells do not get the crucial signal to differentiate during the quiescent stage before differentiation into A1 spermatogonia 18 ; . The ability to stimulate spermatogonial differentiation by suppressing T both in this genetically defective mouse model and in the toxicant-treated rat model implies that similar mechanisms may be involved in the inhibition of spermatogonial differentiation. In the irradiated rat, the reduction in apoptosis occurred well before initiation of differentiation after starting GnRH antagonist treatment 14 ; . In jsd mice, it is not known whether the initial response to GnRH antagonist treatment is reduction in apoptosis of the undifferentiated Aal spermatogonia or initiation of differentiation. The present results indicate that suppression of T with GnRH analogs, which has been proposed for treatment of men rendered azoospermic by cytotoxic treatment for cancer, may also be applicable to human males with idiopathic infertility due to spermatogenic arrest or, more specifically, spermatogonial arrest 19 ; .
